The first wave of AI gave every institution the same model — brilliant, general, and identical everywhere.

Foundation models are extraordinary generalists. They know medicine; they do not know yourmedicine — the consultant's judgement, the institution's protocols, the population it serves. An answer that is the same for every hospital is, by definition, an advantage for none of them.

The next wave belongs to something narrower, and far more powerful: one model per professional, per institution.

Every institution will have its own AI.
