An EMR answers one question precisely: what did we do?

It was never designed to answer the harder one — what happens next. So a practice can hold immaculate records and still lose a third of its patients to silence. Post-operative, mid-titration, due for a six-month screen, the patient waits too, assuming that if something genuinely mattered, somebody at the hospital would surely have called.

Engagement was never in the specification, so it was never in the product.

The record was not built to reach. That is not a design flaw — it is a design gap.
